Office treatment goal

Office treatment target
120–129 / 70–79
mmHg · systolic / diastolic
A threshold defines hypertension; a treatment goal describes BP sought during treatment. Neither is a medication prescription. Both numbers must satisfy a goal; either number can meet a diagnostic threshold. Use averaged, standardized measurements.
The US guideline also encourages systolic BP below 120 in appropriate patients; tolerability and shared decisions matter. ESC prioritizes systolic 120–129 if tolerated; diastolic 70–79 has weaker evidence. Home values are not obtained by automatically subtracting 5 mmHg.
This simplified comparison does not cover pregnancy, acute illness, dialysis, advanced CKD (eGFR <30), or condition-specific treatment. CKD stage, organ damage, PREVENT and SCORE2/SCORE2-OP are not evaluated. Frailty flags prompt review, not a calculated alternative target.
